Joining a clinical trial is a big decision, so it is important to explore all the ways a trial may or may not be a right fit for you. The questionnaire tool below will help you gather your thoughts and opinions about some of the basics of clinical trial participation.

To navigate the questionnaire: Click the next or back arrows to move between the screens. To submit a response, select an answer or enter it in a text box and click the next arrow.
Note: After the introduction screen, each of the 10 questions has its own screen. Follow the instructions on each screen about how to respond and submit your answers. As you go along, the guide will make note of your responses and there will be instructions on how to create a record of your responses at the end.
Once that record is created, you can download it as a PDF to review later or to share with a family member, friend or even as guide when discussing your possible participation with the clinical research team.
